Job summary

Qualifications

Responsibilities

• Arrange post-discharge services.
• Create and follow up on electronic referrals.
• Review patient choice letters with patients/families for signatures.
• Provide follow-up information to Medicare patients prior to discharge.
• Communicate with patients, families, and care team members.
• Complete tasks assigned by the Case Manager.
• Make copies, send faxes and place calls.
• Support process reviews or audits as requested.
