Job summary

PIH Health is seeking a discharge coordination specialist to support the Care Management team by engaging patients and families and handling regulatory discharge documentation. You will assist with post-acute coordination and real-time electronic tracking to ensure timely, compliant discharge planning.

Under delegated authority from the RN Case Manager, this role serves as an operational extension within PIH Health, contributing to a seamless continuum of care across our Whittier network in

• Basic to intermediate computer proficiency.

Responsibilities

• Provides advanced discharge coordination and clerical support for the Care Management team.
