Job summary

Duke Regional Hospital in Durham, NC is seeking a Case Manager to coordinate discharge planning and care transitions across the continuum of care. You will assess patient needs, develop individualized plans, and collaborate with care teams to optimize outcomes.

The role requires a BSN or MSW, at least 3 years of experience, and CCM/ACM/ANCC certification within 3 years. You will support CMS compliance, documentation, and patient education while providing weekend/holiday coverage as needed.

Qualifications

• BSN or MSW required.
• Minimum of 3 years of relevant experience.
• Case Management Certification (ACM, CCM, or ANCC) required within 3 years of hire.
• For BSN-prepared candidates: Must hold a current RN license (or compact license) in the state of North Carolina.

Responsibilities

• Care coordination and discharge planning for assigned patients.
• Develop, implement, and evaluate individualized discharge plans ensuring timely transitions of care.
• Coordinate services such as Home Health, DME, transportation, and post-acute referrals.
• Maintain timely, accurate documentation of assessments and actions; ensure regulatory compliance.
