Job summary

NOR Healthcare Systems Corp. is seeking an LVN Case Manager to support discharge planning and care coordination in a hospital setting.

Under RN supervision, you will prioritize patient needs, collaborate with discharge coordinators, and advocate for patient self-determination while ensuring safe and cost-effective transitions of care. You will perform utilization review, document in the medical record, and coordinate with the interdisciplinary team to optimize timeliness and quality of care.

Qualifications

• Two years of acute hospital case management experience OR completion of Internal Case Management training program.
• Current Licensure as a Vocational Nurse in the State of California.
• Computer/EMR Proficiency and Literacy.
• Knowledge of CMS, Medicare, Medi-Cal and Managed Care reimbursement.
• Familiarity of Joint Commission, CMS, CDPH requirements.
• Excellent written and verbal communication skills in English.
• Ability to establish and maintain effective working relationships across the organization.
• Ability to facilitate and lead interdisciplinary rounds.

Responsibilities

• Collaborates with patient/family to optimize outcomes and coordinate transfers, discharges and transitions of care.
• Applies InterQual or other criteria to determine level of care and ensure timely utilization review.
• Collaborates with patients/caregivers to set goals aligned with physician plans and resources.
• Acts as liaison to medical staff to ensure continuity of hospital services.
• Participates in patient/family meetings and documents informed decision making.
• Contributes data for Utilization Management Committee.
