Job summary

Qualifications

• Two years of acute hospital case management experience OR completion of Internal Case Management training program.
• Current Licensure as a Vocational Nurse in the State of California.
• Computer/EMR Proficiency and Literacy.
• Knowledge of CMS, Medicare, Medi-Cal and Managed Care reimbursement.
• Familiarity with Joint Commission, CMS, CDPH requirements.
• Excellent written and verbal communication skills in English.
• Ability to establish and maintain effective working relationships across the organization.
• Ability to facilitate and lead interdisciplinary rounds.

Responsibilities

• Collaborates with patient/family and health team to optimize outcomes and coord transfers.
• Applies InterQual or approved criteria to determine level of care and LOS.
• Documents utilization and communicates with payer and supervisor.
• Sets goals with patients/caregivers and coordinates discharge planning.
• Acts as liaison to medical staff to ensure continuity of care.
• Participates in patient/family meetings and documents shared decision making.
• Provides data for Utilization Management Committee.
