Northeastern Nevada Regional Hospital, part of Lifepoint Health, is seeking a Part-time RN Case Manager to coordinate discharge planning and ensure quality patient care during hospital stays. The role involves collaboration with physicians, nurses, patients and families to navigate transitions of care and achieve optimal outcomes.

Ideal candidates hold a Nevada RN license, an associate degree (BSN preferred), and current ACLS/BLS certifications.

Qualifikationen

• Nevada RN license required; compact license not available in NV.
• ACLS and Basic Life Support certifications are required.
• One year of experience in a relevant setting is preferred.
• Ability to navigate multiple clinical software systems and communicate effectively.

Aufgaben

• Consult with nursing staff and multidisciplinary team to assess status and appropriateness of care.
• Lead discharge planning and referrals to community resources; coordinate patient transfers.
• Perform chart reviews to address service delivery, outcomes, and reimbursement.
• Communicate with providers, payors, and ancillary services to remove barriers to timely care.
• Document case management plans and outcomes accurately.
• Participate in performance improvement and continuing education.
• May work alternate shifts or be on-call as needed.
