Job summary

Mission Community Hospital in Los Angeles is seeking a compassionate LVN for Case Management to support discharge planning, transition of care, and coordination of post-acute services.

The ideal candidate has SNF experience, a California LVN license, and current BLS certification, with strong organizational and communication skills to collaborate with physicians, nurses, social workers, and the care team.

Qualifications

• Current California LVN license in good standing.
• Minimum of one year of SNF experience.
• Current BLS certification.
• Knowledge of discharge planning, post-acute care and care coordination.
• Strong organizational, communication, and documentation skills.
• Ability to collaborate with physicians, nurses, social workers, and interdisciplinary teams.
• Proficiency in electronic medical records (EMR).

Responsibilities

• Assist the Case Management team with discharge planning and coordination of post-acute care services.
• Coordinate referrals to SNFs, home health agencies, rehabilitation facilities, hospice, DME providers, and other resources.
• Communicate with insurance companies, referral agencies, and healthcare providers to facilitate timely patient transitions.
• Review clinical documentation to support discharge planning and continuity of care.
• Monitor patient progress and identify barriers to discharge.
• Collaborate with physicians, bedside nurses, therapists, and social workers to ensure safe discharges.
• Maintain accurate EMR documentation.
• Assist with prior authorizations and payer coordination as assigned.
• Educate patients and families regarding post-discharge options and care pathways.
• Participate in interdisciplinary rounds and Case Management meetings.
