Job summary

Mission Community Hospital in Los Angeles is seeking a Licensed Vocational Nurse (LVN) to join the Case Management Department. The LVN will support discharge planning, care coordination, and transitions of care for hospitalized patients, ensuring quality outcomes.

The role requires CA LVN license, at least 1 year SNF experience, BLS, strong organizational, communication, and EMR skills, with a Monday–Friday schedule 10:30 AM–7:00 PM and occasional weekend coverage.

Qualifications

• CA LVN license in good standing.
• Minimum of one year SNF experience.
• Current Basic Life Support (BLS) certification.
• Knowledge of discharge planning and post-acute care.
• Strong organizational and communication skills.
• Ability to collaborate with physicians, nurses, social workers.

Responsibilities

• Assist the Case Management team with discharge planning and coordination of post-acute care services.
• Coordinate referrals to SNFs, home health, rehab, hospice and DME providers.
• Communicate with insurers, referral agencies, and healthcare providers to enable timely transitions.
• Review clinical documentation to support discharge planning.
• Monitor patient progress and identify discharge barriers.
• Maintain timely documentation in the EMR and participate in rounds.
