Job summary

Valley Health System seeks a Nurse Case Manager per diem to lead discharge planning through an interdisciplinary team. You will coordinate care plans based on diagnosis, patient needs, and outcomes, and advocate for patients and families.

Responsibilities include timely documentation, collaboration with post-acute providers, and education of the care team on levels of care and regulatory considerations. Alumni with BSN required; Virginia RN licensure essential.

Qualifications

• Bachelor's degree in Nursing required; Master's preferred.
• Graduate of an accredited school of nursing required.
• Virginia RN licensure and BLS/ACLS as applicable.

Responsibilities

• Contribute to goal-directed, age-appropriate plan of care through an interdisciplinary team.
• Develop and document a discharge plan in the most appropriate setting.
• Coordinate plan with post-acute providers and educate team on levels of care.
• Participate in discharge planning rounds and process improvement.
• Complete thorough documentation reflecting discharge status and timelines.
