Responsibilities

• Coordinate discharge planning with residents/patients.
• Act as liaison between patients, physicians and nurses.
• Obtain insurance authorization and manage paperwork for transitions.
• Educate patients and families on care plans and resources.
• Document progress notes weekly in Medical Record.
