Job summary

You will assist in coordinating post-acute care, obtain necessary authorizations, manage transport logistics, and maintain discharge documentation while upholding patient confidentiality and quality service standards.

Qualifications

• High School Diploma or GED is required.
• 1–2 years related experience is preferred.
• Bachelor's degree is a plus.
• Medical terminology knowledge is preferred.

Responsibilities

• Delivers patient notification letters as needed and uploads signatures to the electronic record.
• Assists Case Management team with post-acute care placement and follows through to discharge.
• Obtains authorizations for post-acute care and communicates with hospital, payor, and facilities.
• Coordinates patient transport and manages documentation to ensure successful transport.
• Maintains databases to track discharge barriers and supports expeditious patient transfers.
