Qualifications

• High School Diploma or GED.
• 1-2 year’s related experience.
• Bachelor’s degree preferred.
• Medical terminology knowledge preferred.

Responsibilities

• Delivers patient notification letters and uploads to the electronic record; escalates issues as needed.
• Assists Case Management team with post-acute care placement and follows through until discharge.
• Obtains authorizations for post-acute care and communicates between hospital, payor and facilities.
• Coordinates and documents patient transport to and from hospital and appointments.
• Maintains databases to track discharge barriers and prepares discharge packets for facilities.
