Job summary

Qualifications

• ADN required; BSN preferred.
• 2-3 years of clinical experience.
• 1-2 years of case management experience preferred.
• RN license in DC or MD required.
• CCM certification preferred.

Responsibilities

• Collaborate with multidisciplinary team to develop and coordinate the plan of care.
• Communicate with patient, family, and care team across settings.
• Monitor care and outcomes; adjust the care plan as needed.
• Manage caseload and participate in Performance Improvement initiatives.
• Ensure compliance with regulations and documentation standards.
