Job summary

The University of Maryland Medical System is seeking a Care Management Technician to support discharge planning and care coordination under supervision of an RN Case Manager or Social Worker.

You will create discharge packets, arrange transportation, deliver DME, and communicate with patients and families using email, phone, and EMR systems. CNA preferred.

This role requires strong communication, organizational skills, and the ability to work under pressure in a hospital setting.

Qualifications

• Strong communication, written and interpersonal skills are required.
• Customer service experience is preferred.
• Experience in hospital or health care is advantageous.

Responsibilities

• Create discharge packets with documentation from multiple disciplines.
• Deliver DME and assist with insurance/charity paperwork.
• Confirm start of home care and coordinate transportation for discharge.
