Job summary

University of Maryland Medical System is seeking a Case Manager to provide care coordination and discharge planning for an assigned patient caseload. Under general supervision, you will assess patient needs, coordinate plans of care, and monitor outcomes across the continuum of care.

Responsibilities include multidisciplinary collaboration with physicians, nursing, social work, and other departments; development of discharge plans; ensuring timely authorizations; and supporting efforts to reduce

Qualifications

• Licensure as a Registered Nurse in Maryland is required.
• Two years of Acute Care hospital experience preferred.

Responsibilities

• Assess assigned patients and determine appropriate level of care upon disposition.
• Coordinate and implement Discharge Plan A and alternative Plan B with documentation.
• Collaborate with physicians, nursing, social work, and other disciplines to remove barriers to care.
• Lead care management rounds and contribute to performance improvement.
• Ensure timely authorizations and reduce avoidable days.
