Job summary

Paycom is seeking a Coordinated Care Specialist to provide disaster and crisis case management across Harris, Fort Bend, Waller and Montgomery Counties from Houston, TX. Responsibilities include needs assessments, individual stabilization plans, and referrals to community resources.

The role involves in-person and in-office visits, maintaining client contact, and ensuring timely, high-quality services through collaboration with delivery partners.

Qualifications

• Bachelor's degree in Behavioral Sciences, Human Services, or other Social Services field is required.
• Three (3) years of experience in case management or related client services.
• Bilingual English/Spanish communication preferred.
• Strong communication and organizational skills.

Responsibilities

• Conduct outreach and intake; develop stabilization plans and referrals.
• Perform home and community visits; document progress and updates.
• Maintain client files with comprehensive case history and notes.
• Collaborate with partners on disaster/crisis response; coordinate services.
