Job summary

United Way of Greater Houston seeks a Coordinated Care Specialist to join the Strike Team, delivering disaster and crisis case management across Harris, Fort Bend, Waller and Montgomery Counties. You will complete Needs Assessments, develop Individual Stabilization Plans, and refer to community services with partners.

You will conduct outreach, home and community visits, and ensure timely plan completion while maintaining client records and coordinating with professionals for optimal outcomes.

Qualifications

  • Bachelor's degree required in Behavioral Sciences, Human Services, or related field.
  • Three years of experience in case management or direct social services.
  • Bilingual English/Spanish preferred.

Responsibilities

  • Conduct needs assessments and develop Individual Stabilization Plans.
  • Perform home and community visits to provide ongoing support and access to community resources.
  • Assist clients in identifying and obtaining available benefits and services.
  • Coordinate with family services, medical/psychological professionals and other contacts post-disaster.
  • Answer 211 calls and maintain the statewide resources database.
  • Prepare reports for Manager and AVP; contribute to program evaluation and growth.
  • Maintain thorough documentation in physical and digital files and attend workshops for professional development.
