United Way of Greater Houston seeks a Coordinated Care Specialist to join the Strike Team, delivering disaster and crisis case management across Harris, Fort Bend, Waller and Montgomery Counties. You will complete Needs Assessments, develop Individual Stabilization Plans, and refer to community services with partners.

You will conduct outreach, home and community visits, and ensure timely plan completion while maintaining client records and coordinating with professionals for optimal outcomes.

Qualifications

  • Bachelor's degree required in Behavioral Sciences, Human Services, or related field.
  • Three years of experience in case management or direct social services.
  • Bilingual English/Spanish preferred.

Responsibilities

  • Maintain thorough documentation in physical and digital files and attend workshops for professional development.

Job description

The Coordinated Care Specialist is a member of the United Way Strike Team and will provide disaster and crisis case management services to individuals and families impacted by disasters and/or crisis events throughout Harris, Fort Bend, Waller and Montgomery Counties. The Specialist is responsible for completing Needs Assessments, developing Individual Stabilization Plans, and making appropriate community referrals for additional services with service delivery partners. The Specialist will maintain appropriate contact with the client and ensure quality services are provided, up to and including in-person and in-office visits, and that their Individual Stabilization Plan is completed in the most time efficient way possible.PRINCIPAL DUTIES AND RESPONSIBILITIES:Conduct client outreach and complete eligibility and intake process for each client. Conduct home and community visits with the client to provide on-going support and assistance in applying for and obtaining community resources to meet disaster/event caused unmet needs. Support client in the development of their Individual Stabilization Plan. Conduct follow-up visits to monitor and assess the effectiveness of services provided. Document comprehensive client background information, case history, updates, and progress towards Individual Stabilization Plan in both physical and digital files via online software.Assist clients in identifying and obtaining available benefits as well as social and community resources. Serve as liaison between family services, medical/psychological health professionals and other contacts to help individuals and families in post-disaster events. Answer 211 calls to maintain familiarity with caller issues and concerns as well as the 211 statewide database of resources.Prepare reports as requested by Manager. Provide on-going program evaluation and recommendations to the Manager and AVP for continuous growth and quality. Provide reports and status for all cases in their portfolio as needed by program demands. Participate in workshops, seminars, education programs and other activities that promote professional growth and development. Support community education and outreach efforts of the department.Other duties as assigned. As an essential employee, may be required to work extended hours and/or times other than standard business hours including on-call rotations.EDUCATION:Bachelor's degree in Behavioral Sciences, Human Services, or other Social Services field is required.SKILLS AND EXPERIENCE:Three (3) years of experience in case management, direct social services, or other client casework, including working within multi-disciplinary teams to develop case plans with specified goals and outcomes.Experience in emergency disaster response and/or recovery environments, including working within multi-disciplinary teams to develop case plans with specified goals and outcomes, is strongly preferred.Bilingual (English / Spanish) communication skills preferred.Strong communication skills, both written and verbal, are critical.Demonstrated ability to organize and prioritize workloads and to meet deadlines.Demonstrated ability to handle multiple projects concurrently with minimal supervision and handle confidential information.Strong orientation to detail, excellent organizational and documentation skills.
