Coordinated Care Specialist # Coordinated Care SpecialistHouston, TX 77007## OverviewSalary Range$48,500.00 - $52,000.00 Salary/yearPosition TypeFull TimeTravel PercentageUp to 50%## DescriptionThe Coordinated Care Specialist will provide emergency case management services to individuals and families impacted by disasters and/or crisis events throughout Harris, Fort Bend, Waller, and Montgomery Counties. The Specialist is responsible for completing needs assessments, developing individual stabilization plans, and making appropriate community referrals to service delivery partners for additional services. The Specialist will maintain appropriate contact with the client and ensure that quality services are provided, including in-person and in-office visits, and that the client's individual stabilization plan is completed as efficiently as possible.### **PRINCIPAL DUTIES AND RESPONSIBILITIES:*** Conduct client outreach and complete eligibility and intake process for each client. Conduct home and community visits with the client to provide on-going support and assistance in applying for and obtaining community resources to meet disaster/event caused unmet needs. Support client in the development of their Individual Stabilization Plan. Conduct follow-up visits to monitor and assess the effectiveness of services provided. Document comprehensive client background information, case history, updates, and progress towards Individual Stabilization Plan in both physical and digital files via online software.* Assist clients in identifying and obtaining available benefits as well as social and community resources. Serve as liaison between family services, medical/psychological health professionals and other contacts to help individuals and families in post-disaster events. Answer 211 calls to maintain familiarity with caller issues and concerns as well as the 211 statewide database of resources.* Prepare reports as requested by Manager. Provide on-going program evaluation and recommendations to the Manager and AVP for continuous growth and quality. Provide reports and status for all cases in their portfolio as needed by program demands. Participate in workshops, seminars, education programs and other activities that promote professional growth and development. Support community education and outreach efforts of the department.* Other duties as assigned. As an essential employee, may be required to work extended hours and/or times other than standard business hours including on-call rotations.## Qualifications### **EDUCATION:**Bachelor's degree in behavioral sciences, human services, or other social services field is required. ### **EXPERIENCE:**Three (3) years of experience in case management, direct social services, or other client casework, including working within multi-disciplinary teams to develop case plans with specified goals and outcomes. Experience in emergency response and recovery environments is preferred. Exceptional organizational and project management skills, along with strong attention to detail, are required. Strong written and verbal communication skills are required, and bilingual (English/Spanish) communication skills are preferred.
