Job summary

Momentum Community Connections is seeking a Disability Support Worker to deliver support services that enhance living, social and recreational skills for people with a disability. The role focuses on inclusion, independence, personal choice and tailored support to meet individual needs in a community setting.

The position involves assisting with daily tasks, promoting participation, and empowering participants to engage in decision-making. A casual, on-site role with award rates applies.

Qualifications

  • First Aid and CPR certifications are essential or willing to obtain within six months.
  • Minimum 6 months relevant industry experience.
  • Driver’s license and a reliable, insured vehicle required.
  • Police checks (including DWES and WWCC) are required.
  • NDIS Worker Orientation Module Certificate required or to be obtained.

Responsibilities

  • Provide personal support and assist with household tasks as per the participant plan.
  • Encourage participation in tasks and community activities to reach goals.
  • Support empowerment and active engagement in daily activities and decisions.
  • Create a safe, enjoyable environment and foster participant growth and dignity.
  • Maintain proactive communication and perform administrative duties as needed.

About The Position

The Disability Support Worker is responsible for delivering support services aimed at enhancing the living, social and recreational skills of people with a disability. Community inclusion, independence, decision making, and personal choice should be promoted through these supports, with a focus on individual needs.

Key Responsibilities Include, But Are Not Limited To
  • Personal Support
    • Provide assistance with households as outlined in the participants plan and encourage them to participate in those tasks actively.
    • Provide assistance with community participation for our participants to reach their goals and objectives.
  • Participant Empowerment
    • Provide support that will enable participants to actively engage in the community or in any daily personal activities of their own choice and interest
    • Create a safe, enjoyable and creative environment at all times
    • Maintain an environment which promotes participants decision making skills and contribute to their personal growth
    • Show empathy and respect for the dignity, rights, and care of all the participants
    • Actively always engage with participants

WHAT DO YOU NEED?
Essential
  • First Aid Certificate and CPR (or willing to obtain within six months)
  • Cert III or IV Individual Support, Mental Health, Aged Care, Community Services, Disability Work or similar is preferred (or willing to obtain one)
  • Minimum 6 months relevant industry experience
  • Driver’s License
  • Reliable and comprehensively insured vehicle
