Job summary

Claro Disability Services in New York, Village of Hilton, seeks compassionate support workers for people with complex needs. You will provide person-centered care, assist with mobility and daily tasks, and coordinate with families and medical teams to ensure smooth care delivery.

Shifts are part-time, with a 13-hour weekly schedule including Tue/Wed/Thu 12:00–15:00 and Fri 17:00–21:00, offering meaningful, community-focused work with paid induction and travel between client shifts.

What you’ll do
  • Person-centered planning: Chat with the client (and family/advocates), get the know them and understanding of goals and routines.
  • Complex physical care: Help with mobility, transfers, positioning, daily tasks, and devices (wheelchairs, aids, etc.) as outlined in the plan
  • Medical supports: Watch for changes, follow med/clinical plans, and liaise with the care team if things shift.
  • Social time and participation: Bring meaning, fun, and community access aligned with interests.
  • Autonomy and advocacy: Support decision‑making, respect consent, boost independence where possible.
  • Cultural humility: Respect backgrounds, beliefs, and ways of being

Hours
  • Tuesday 1200 - 1500
  • Wednesday 1200 - 1500
  • Thursday 1200 - 1500
  • Friday 1700 - 2100 (Community access)
