Qualifications

• Bachelor’s degree in human services and at least one year of disability services delivery.
• Valid driver’s license, reliable transportation, and adequate liability insurance.
• Ability to work independently or collaboratively within a team.

Responsibilities

• Process requests for services by completing intakes and eligibility documents.
• Provide information regarding service options.
• Draft funding requests and monitor ongoing funding needs.
• Determine initial and continuing client eligibility and maintain case records.
• Coordinate, implement, and monitor the Individual Service Plan.
• Advocate for clients and facilitate involvement of the Interdisciplinary Team.
• Establish and maintain relationships with clients, counties, and service providers.
• Participate in agency and state-wide meetings and trainings.
