Opportunity Builders, Inc. is seeking a case manager to provide participant-centered case coordination and advocacy for individuals with disabilities. You will work with the PCP planning team, coordinate with CCSs, and help families navigate services and funding sources including DDA, DORS, and schools.

A salary range of $50K–$58K is offered. The role requires strong communication, documentation discipline, and collaboration with multiple stakeholders.

Qualifikationen

• 1 year of experience with individuals with disabilities and case coordination or QA/compliance.
• High school diploma or equivalent required; bachelor’s degree preferred.
• Excellent written and oral communication skills.

Aufgaben

• Provide case coordination to participants to ensure quality care and PCP process.
• Collaborate with CCSs to facilitate PCP meetings.
• Establish person-centered goals and include them in SIP.
• Maintain documentation and progress in OBI systems.
• Request and secure DDA funding and ensure regulatory compliance.
• Train staff on participant-specific requirements and ensure training documentation.
