Job summary

Rappahannock Area Community Services Board seeks a case manager to coordinate services for individuals with developmental disabilities. You will assess needs, develop ISP, link to necessary supports, and advocate for appropriate interventions within a collaborative care team.

Responsibilities include arranging transportation, crisis support, and documenting services to ensure quality care and insurance reimbursement across platforms.

Qualifications

• Bachelor's Degree in a behavioral health field and professional work experience involving case management for individuals with developmental disability including some program management and supervisory experience.
• Knowledge of developmental disability causes and best practices in supporting individuals.
• Understanding of service planning, ISP components, and documentation standards.
• Ability to advocate for appropriate interventions and coordinate with diverse providers.

Responsibilities

• Assesses needs to determine appropriateness for intellectual disability services and develops individual service plans.
• Links the individual to services and supports specified in the ISP.
• Assists the individual in locating or obtaining needed services and crisis resources.
• Arranges transportation for individuals or families to access services.
• Coordinates services and benefits with other agencies and providers involved.
• Provides crisis intervention and supportive services to family members.
• Participates on treatment teams and advocates for appropriate treatment.
