Job summary

Adams County Government seeks a professional to provide intermediate-to-advanced human services support to adults with disabilities and older adults. You will deliver information, counseling, and enrollment assistance for long term care programs, including Medicaid, under supervision from the ADRC team.

Responsibilities include outreach, coordination, advocacy, and collaboration with community providers to improve access and outcomes.

Qualifications

• Bachelor's degree in social work, sociology, human services or related field.
• Obtain Adult Long Term Care Functional Screener certification within six months of hire.
• Valid driver's license in Wisconsin.

Responsibilities

• Provide customer driven information and assistance via phone, email, or in person on local resources.
• Offer option counseling to meet long term care needs and decisions.
• Assist individuals with functional and financial access to publicly funded long term care programs.
• Assist individuals with Medicaid application process.
• Provide enrollment/disenrollment information for long term care programs.
• Provide short term care coordination for individuals with complex needs.
• Participate in marketing and outreach activities.
• Collaborate with other service providers and community organizations.
• Advocate for individuals and groups to address service gaps and improve access or quality.
• Perform other related duties as assigned.
