Websitecommerce is seeking a Case Manager in Minnesota to provide service coordination for individuals with disabilities and their families. You will support CADI, BI, and DD programs, focusing on person-centered planning and collaboration with care teams.

The role requires a bachelor’s degree in human services, travel to client homes, and readiness for flexible hours including some evenings and weekends. Emphasis on empowerment, documentation, and high-quality service delivery.

Formación

• Bachelor’s degree in human services required.
• Experience in disability services preferred.
• Proficiency with MS Office.
• Ability to maintain tracking systems for multiple tasks.
• Excellent written and verbal communication with clients and professionals.
• Must be willing to work flexible hours including evenings/weekends.

Responsabilidades

• Coordinate services for individuals with disabilities.
• Identify needs and preferences of individuals.
• Find resources to meet needs.
• Complete referrals for services.
• Maintain regular contact with providers for quality service.
• Manage payment authorization for providers.
• Develop and maintain annual support plans.
• Document case notes in the electronic record system.
• Conduct in-person home visits as scheduled.
