Job summary

Meridian Services is seeking a Case Manager/Social Worker to help people with disabilities and the elderly access resources and services that fit their needs. You will advocate for persons served and manage caseloads including individuals with intellectual disabilities, traumatic brain injuries, CADI diagnoses, and elderly/alternative care.

The role is remote within Otter Tail County MN with mandatory state travel; after 6 months you may work from home up to 2 days/week.

Qualifications

• Must be licensed Social Worker (BSW required for licensure) or licensed Graduate Social Worker (MSW required for licensure) as per Minnesota Board.
• Or have a 4-year degree in Human Services, Psychology or Sociology or related field.
• Successful clearance of Department of Human Services background check.
• Successful clearance of Motor Vehicle Background Check and acceptable driving record per policy.

Responsibilities

• Visits with persons on caseload and attendance at team meetings.
• Completing or attending needs assessments; developing individual service plans (CSSP).
• Assisting with planning for new service development.
• Locating residential, vocational, and other needed services for each person’s needs.
• Monitoring service delivery and ensuring health and safety needs are met.
• Completion of referrals and crisis intervention as needed.
• Developing waiver budgets.
• Completing case notes.
• Acting as an advocate for the people we provide services to.
