Job summary

HAMMER RESIDENCES INC is seeking a Case Manager in Minnesota to provide service coordination for individuals with disabilities. The role focuses on person-centered planning, linking clients to community resources, and supporting families within CADI, BI, and DD programs.

Primary duties include developing annual support plans, advocating for client health and safety, and conducting home visits. Flexible hours and travel across the Twin Cities area are required; a reliable vehicle is necessary.

Qualifications

• Bachelor’s Degree in a human services field is required by DHS.
• Experience in disability services preferred.
• Experience with mental health is preferred for CADI case management.
• Must have a reliable vehicle and driving record to support in-person visits.
• Proficiency with Microsoft Office Products (Word, Excel, Outlook).

Responsibilities

• Provide person-centered service coordination to identify needs and connect resources.
• Develop and maintain annual support plans with policy components.
• Advocate for health, safety, and independence of individuals.
• Conduct minimum two in-person home visits annually and travel as needed.
• Document case notes in electronic records with time-stamped entries.
• Maintain documentation integrity across all tasks.
