Job summary

WelbeHealth is seeking a Program Director to lead PACE operations and ensure high-quality care for seniors at home. Reporting to the Executive Director, you will oversee nutrition, social work, day center, personal care, scheduling, and participant experience.

Ideal candidates hold a relevant bachelor’s degree (master’s preferred) with 7+ years of senior-health program leadership, plus 3+ years supervising. CPR/First Aid certification is required within 180 days of hire.

Qualifications

• Bachelor’s degree in health administration or healthcare management, or a related field.
• Five or more years of management experience may be substituted; master’s degree preferred.
• Experience in nursing, social work, psychology, or related rehab fields is required.
• Seven years of experience with at least three in a supervisory capacity in a senior health program.
• Multi-disciplinary management experience; value-based care preferred.
• CPR and First Aid certification within 180 days of hire and maintained thereafter; certification on hire preferred.

Responsibilities

• Oversee critical PACE functions, including hiring, training, supervising, mentoring staff and managing workflows.
• Develop and facilitate an effective Interdisciplinary Team (IDT) and related meetings with documented follow-up.
• Oversee SOP adherence and regulatory requirements across the program.
• Collaborate to achieve high participant outcomes across home care, health services, transportation, and calls support.
• Manage program site, supplies, equipment, maintenance and vendor oversight.
