Job summary

WelbeHealth PACE is seeking a Registered Dietitian to lead the nutrition services program, conducting assessments and counseling participants while coordinating with food and supplement providers to ensure compliant menus and delivery.

The RD will collaborate with nursing, therapy, and other IDT members to create holistic nutrition plans central to participant care, and will oversee menu planning, meal delivery, and staff mentoring.

Qualifications

• Registered Dietitian with at least two (2) years of experience directing nutrition services in a healthcare setting.
• Current Basic Life Support (BLS) and First Aid certification required.
• Excellent leadership, organization, and communication skills in settings with seniors, their families, and IDT members.
• At least one (1) year of experience working with a frail or elderly population.

Responsibilities

• Conduct nutrition assessments to determine participant needs, preferences, and goals.
• Actively participate in IDT meetings to develop participant care plans.
• Deliver and document nutrition interventions in accordance with physician orders or care plans, including specific diets and counseling.
• Oversee menu planning and meal delivery programs to meet safety and regulatory requirements and diet specifications.
• Manage the nutrition staff, including hiring, training, overseeing, and mentoring.
• Guide the intake and enrollment process to move prospective participants through enrollment efficiently.
