Job summary

WelbeHealth PACE provides seniors with the opportunity to continue living in their homes and in their communities. The Medical Director I performs in-person comprehensive assessments and coordinates with the IDT to develop a care plan for each participant.

Under the Sr. Medical Director, the Medical Director I oversees all care of participants in nursing homes and hospitals, including admits and discharges for acute, short-term, and long-term placement, and the use of medical specialists to

Qualifications

• Residency program accredited by ACGME
• Master’s degree or doctorate required
• Board Certification – Internal Medicine or Family Practice
• Board Certification – Geriatrics preferred
• Active California Physician License
• Active CDS Certification and DEA
• Must be enrolled or eligible to provide services to Medicare/Medi-Cal participants
• CPR and First Aid certification within 180 days of hire

Responsibilities

• Provide assessment, planning, primary care, and continuous medical coverage to participants
• Collaborate with community-based physicians and IDT in discharge planning
• Active participation in IDT and communication of participant changes
• Coordinate 24-hour care delivery and oversee care in nursing homes and hospitals
• Take part in rotating night and weekend call

Education

Residency program accredited by ACGME

Master’s degree or doctorate degree

Board Certification – Internal Medicine or Family Practice

Board Certification – Geriatrics, preferred

Active California Physician License

Active CDS Certification and DEA

Enrollment or eligibility for Medicare/Medicaid services
