Job summary

Complete Care is seeking a Director of Social Services (LSW/CSW) for a full-time day shift in New Jersey. The role oversees the Social Services Department, leads program development, and supports residents and families throughout the healthcare journey.

You will participate in care planning, ensure compliant documentation, and collaborate with the clinical team. A supervisory background and strong communication skills are essential for success in this leadership position.

Qualifications

• Current LSW or CSW required.
• Long-term care experience preferred, but not required.
• Prior supervisory or management experience preferred.
• Current AHA BLS CPR certification recommended.
• Strong communication, organizational, and documentation skills.
• Computer proficiency required.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Lead and oversee the Social Services Department at Complete Care.
• Develop and implement social service programs, policies, and goals.
• Advocate for residents' psychosocial, emotional, and behavioral health needs.
• Participate in interdisciplinary care plan meetings and collaborate with the clinical team.
• Maintain accurate, timely, and compliant documentation.
• Educate residents and families regarding Advance Directives and available resources.
• Coordinate admissions, transitions, and discharge planning to ensure positive outcomes.
• Provide guidance and support to social services staff.
• Assist residents and families with community resources and support services.
• Perform additional duties as assigned by the Administrator.
