Job summary

Complete Care in New Jersey is hiring a Director of Social Services (LSW/CSW) to lead the Social Services Department and develop programs that support residents and their families. This full-time role includes overseeing discharge planning, advocacy for resident well-being, and collaboration with the clinical team.

The candidate should hold an LSW or CSW, with preferred long-term care and supervisory experience, and strong communication, documentation, and computer skills.

Qualifications

• Current LSW or CSW required.
• Long-term care experience preferred.
• Prior supervisory or management experience preferred.
• BLS CPR certification recommended.
• Strong communication, organizational, and documentation skills.
• Computer proficiency required.
• Ability to work independently and as part of an interdisciplinary team.

Responsibilities

• Lead and oversee the Social Services Department at Complete Care.
• Develop and implement social service programs, policies, and goals.
• Advocate for residents' psychosocial, emotional, and behavioral health needs.
• Participate in interdisciplinary care plan meetings and collaborate with the clinical team.
• Maintain accurate, timely, and compliant documentation.
• Educate residents and families regarding Advance Directives and available resources.
• Coordinate admissions, transitions, and discharge planning to ensure positive outcomes.
• Provide guidance and support to social services staff.
• Assist residents and families with community resources and support services.
• Perform additional duties as assigned by the Administrator.
