Job summary

Complete Care is seeking a Director of Social Services to lead the social work team at our New Jersey facility.

LSW or CSW is required; prior long-term care and staff management experience are preferred. You will direct policies, ensure documentation accuracy, participate in interdisciplinary care planning, and educate residents and families.

This full-time role includes a $5,000 sign-on bonus, comprehensive benefits, and opportunities for growth within a compassionate, team-driven environment.

Qualifications

• Must have LSW or CSW
• Previous long term care experience is preferred
• Prior experience managing social work staff is preferred
• Directs, establishes & plans the overall policies/goals for the Social Services Dept
• Maintain accurate, thorough, and timely documentation
• Participates in interdisciplinary care plan meetings to ensure treatment of the social and emotional needs of the resident
• A wide degree of creativity and latitude is expected
• Review Advanced Directives and provide education to residents and family members as needed
• Help provide a smooth admission and discharge process for residents and family members
• Ability to work independently or part of an interprofessional team
• Computer knowledge and experience using Point Click Care
• Other duties as assigned by Administrator

• Works independently or as part of an interprofessional team
• Uses Point Click Care for documentation and care planning
• Performs other duties as assigned by Administrator
