Qualifications

• Bachelor’s or Master’s degree in Social Work (BSW/MSW) or related field preferred.
• Social Service Designee certification or will to obtain.
• 1 year of experience in social services, preferably in an LTC/SNF or similar healthcare setting.
• 1 year of management/supervising experience.
• Strong knowledge of social work principles, ethics, and regulations.
• Excellent communication, leadership, and problem-solving skills.
• Ability to work collaboratively with a multidisciplinary team.

Responsibilities

• Develop and implement social service programs that meet the needs of residents/clients.
• Provide counseling, support, and advocacy for individuals and families.
• Collaborate with medical and administrative staff to enhance holistic care.
• Ensure compliance with state, federal, and industry regulations.
• Maintain accurate documentation and reports on services provided.
• Establish relationships with community agencies and resources.
• Train and supervise social work staff (if applicable).
