Job summary

Green Oaks of Park Forest is seeking a nursing leader to oversee clinical operations and staff in Park Forest, IL. The role emphasizes assessments, care planning, wellness services, and collaborative care with physicians.

The candidate should have an LPN license (RN preferred) and at least two years of supervisory nursing experience in supportive living, with excellent communication and leadership skills to mentor the team.

Qualifications

• Current state nursing licensure required (LPN); RN preferred.
• Minimum of 2 years nursing supervisory experience.
• Prior senior/supportive living experience strongly preferred.
• Excellent written and verbal communication skills.
• Must possess leadership and supervisory ability; able to mentor and inspire team.

Salary Range: $97,500.00 - $105,000.00 Salary/year

Job Shift: 1st shift

Responsibilities

• Conducts initial assessment of prospective residents, including off-site visits


• Performs wellness assessments and develops individualized care plans based on the needs and preferences of each resident


• Delivers health promotion services, offers disease prevention counseling, and provides self-care education to residents


• Engages with residents and family members in a compassionate and impactful manner, creating a lasting and positive experience


• Fosters communication by regularly engaging with physicians, pertinent healthcare providers, and family members


• Hires, trains, schedules, and leads the clinical team in providing exceptional care experiences


• Coordinates the Community's infection control program while maintaining state regulatory compliance


• Develop and oversee the Community's medication management program


• Contributes toward quality assurance and performance improvement initiatives


• Complies with all rules and regulations regarding confidentiality and privacy of resident information

Qualifications

• Maintains current state nursing licensure. LPN required, RN preferred


• Minimum of two (2) years nursing supervisory experience


• Prior senior/supportive living experience strongly preferred


• Excellent written and verbal communication skills


• Must possess leadership and supervisory ability and the readiness to mentor, motivate, and inspire team members


• Demonstrates exceptional interpersonal skills and ability to serve with love, compassion, and dignity
