Job summary

Heartland Alliance Health seeks a Manager of Housing and Health Integration in Chicago to lead two program areas—Community Based Care and HUD-funded Chronic Homeless Supportive Housing. You will supervise staff, align care coordination with no-wrong-door policy, and ensure high-quality, trauma-informed services for people experiencing homelessness.

The role requires a master's degree in a clinical field, active Illinois licensure, and 3+ years in care coordination with at least 1 year

Qualifications

Master's degree in Social Work, Counseling, Psychology, or a related clinical field.
Active, unrestricted independent clinical license in Illinois required (LCSW, LCPC, or equivalent).
Minimum 3 years of experience in case management, care coordination, or social services.
At least 1 year in a supervisory or lead capacity.
Experience working with people experiencing homelessness, chronic homelessness, and/or serious mental illness strongly preferred.
Experience with HUD-funded supportive housing programs and/or HIV/AIDS services preferred.

Responsibilities

Staff Supervision and Team Management: supervise, coach, and develop assigned staff.
Program Oversight and Service Delivery: ensure low-barrier, trauma-informed care and linkage to care.
Compliance and Reporting: ensure funder requirements and HIPAA compliance are met.
Documentation: oversee data entry and reporting in CaseWorthy and Athena Practice.
