Job summary

Future Transitions, Inc. seeks a Direct Support Professional 1 in Lancaster, CA to assist adults with developmental disabilities in home and community settings. This part-time to full-time role emphasizes personal care, daily living activities, and community integration.

Responsibilities include documenting daily notes, coordinating transportation and appointments, ensuring safety, and collaborating with DSP 2/3 and the Area Coordinator as outlined in the ISP and Daily Task List.

Qualifications

• One year of experience providing care to persons with developmental disabilities.
• Experience working with seniors in a human service field is also acceptable.
• High School Diploma or equivalent preferred.

Responsibilities

• Assist or perform for the consumer in personal care, such as grooming, feeding, toileting, meal preparation.
• Support and guide the consumer in mobility, community access, and learning new skills.
• Assist or facilitate medical appointments, communication, equipment repair, and schedule travel and activities.
• Provide companionship, guidance and supervision during Community activities.
• Record daily in the consumer logs and notes.
• Report changes in the consumer’s condition to supervisor.
• Ensure health and safety during shifts and assist with medications as needed.
• Maintain the consumer’s home and adaptive equipment; report damages immediately.
• Perform duties as specified by the consumer, DSP 2/3 and Area Coordinator per ISP.
