Qualifications

• Candidates must be able to read, write, and comprehend English to accurately follow care plans.
• Experience in providing training and case management to adults with intellectual disabilities.
• MAP Certification and CPR/First Aid are required training.

Responsibilities

• Assist individuals with daily living activities such as personal hygiene and meal preparation.
• Transport individuals to appointments using wheelchair accessible vans.
• Complete Individual Support Plans to promote personal goals and independence.
