Job summary

The New York Foundling seeks a Direct Support Professional in New York to empower individuals with developmental disabilities through personalized care and skill‑building activities. You will support daily living, accompany appointments, and help promote independence within a compassionate team.

As part of a safety‑driven, person‑centered program, you’ll document progress, collaborate with healthcare professionals, and advocate for clients’ choices and needs.

Qualifications

• High school diploma or equivalent; direct care or human services experience preferred.
• Valid driver’s license with up to 6 points and at least 6 months experience.
• Strong interpersonal and communication skills with patience and empathy.
• CPR/First Aid certification preferred; additional training in disabilities or healthcare is a plus.
• Commitment to compassionate care, dignity, and respect for individuals with developmental disabilities.
• Ability to adapt to changing needs and environments with problem‑solving skills.
• Maintain confidentiality and comply with agency policies and procedures.
• Demonstrate a strong commitment to safety while supporting individuals.

Responsibilities

• Implement and monitor plans of care for persons supported, including assessments and life plans.
• Provide direct care such as daily living activities, medication administration, and transportation assistance.
• Facilitate community outings, social events, and healthcare follow‑ups.
• Use person‑centered approaches and positive behavior support strategies to address challenging behaviors.
• Maintain accurate records, progress notes, and incident reports per policy.
• Collaborate with interdisciplinary teams to develop and implement care plans.
• Advocate for rights, choices, and preferences of individuals, promoting self‑advocacy.
• Be a custodian and mandated reporter as required by policy.
