Job summary

You will follow person-centered plans, document progress, collaborate with healthcare teams, and advocate for clients while maintaining safety and confidentiality.

Qualifications

• High school diploma or GED required; experience in direct care preferred.
• Valid driver’s license from NY/NJ/CT/PA with ≤6 points.
• Strong interpersonal and communication skills.
• CPR and First Aid certification preferred.
• Commitment to compassionate care and dignity for individuals with developmental disabilities.

Responsibilities

• Implement and monitor plans of care for persons supported, including various plans of care.
• Provide direct care: personal hygiene, meals, medication administration, and transportation.
• Facilitate community outings and accompany appointments.
• Use person-centered approaches to address challenging behaviors and ensure safety.
• Maintain records: progress notes and incident reports per agency policies.
• Collaborate with interdisciplinary teams and families to develop comprehensive care plans.
• Advocate for rights and choices of individuals, promoting self-advocacy and empowerment.
• Administer medications as prescribed and monitor for adverse reactions.
