Job summary

Qualifications

• High school diploma or equivalent; experience in direct care preferred.
• Valid driver’s license with 6 months of experience and ≤6 points.
• Strong interpersonal and communication skills; patient and empathetic.
• CPR/First Aid certification preferred; additional training in developmental disabilities a plus.
• Commitment to compassionate care and dignity for individuals with developmental disabilities.
• Ability to adapt to changing needs and environments.

Responsibilities

• Implement and monitor care plans for persons supported (plans can include CFAs, Life Plans, treatment plans, behavior supports, etc).
• Provide direct care and assistance in daily living activities, including hygiene, meals, medication administration, and transportation.
• Facilitate community outings, recreational activities, and social events; accompany appointments and follow-ups.
• Address challenging behaviors using person-centered approaches and safety-focused strategies.
• Maintain accurate records, progress notes, and incident reports per agency policies.
• Collaborate with healthcare professionals, therapists, and families to develop comprehensive care plans.
• Advocate for rights and preferences of individuals; custodial duties and mandated reporting per policy.
• Administer medications as prescribed and monitor for adverse reactions.
