Job summary

The New York Foundling is seeking a Direct Support Professional in New York, NY. This role focuses on empowering individuals with developmental disabilities through personalized support, social and daily living skills development, and participation in community activities.

You will document progress, collaborate with care teams, and advocate for clients while ensuring safety and adherence to regulations. A valid driver’s license and CPR/First Aid are preferred.

Qualifications

• High school diploma or equivalent; direct care experience preferred.
• Valid NY/NJ/CT/PA driver’s license with <=6 points.
• Strong interpersonal and communication skills; patience, empathy and a desire to support persons with developmental disabilities.
• CPR and First Aid certification preferred; additional training in developmental disabilities or healthcare is a plus.
• Commitment to providing compassionate care and promoting dignity and respect for individuals with developmental disabilities.
• Ability to respond effectively to changing needs and environments, showing flexibility and problem-solving skills.

Responsibilities

• Implement and monitor varying plans of care for the persons supported, including assessments and plans such as Life Plan and Behavior Support Plans.
• Provide direct care and assistance with daily living activities, including personal hygiene, meals, medication administration, and transportation.
• Facilitate community outings and social events to promote inclusion and independence; accompany to appointments and healthcare follow-ups.
• Use person-centered approaches and positive behavior support to address challenging behaviors and maintain a safe, nurturing environment.
• Maintain accurate records, daily progress notes, and incident reports to track achievements, behaviors, and health status in compliance with policies.
• Collaborate with interdisciplinary teams, including healthcare professionals and family members, to develop comprehensive care plans.
• Advocate for rights, choices, and preferences of individuals with developmental disabilities; be a custodian and mandated reporter in accordance with OPWDD and policies.
• Administer medications as prescribed and monitor for adverse effects.
