Job summary

Qualifications

• High school diploma or equivalent required.
• Valid driver’s license with up to 6 points preferred.
• Strong interpersonal skills, patience, and empathy.
• CPR and First Aid certification preferred; additional disability training is a plus.

Responsibilities

• Implement and monitor plans of care for persons supported.
• Provide direct care in daily living activities and assist with medications.
• Facilitate community outings and social activities to promote inclusion.
• Record progress notes and incident reports; ensure regulatory compliance.
• Collaborate with interdisciplinary teams and families on care plans.
• Advocate for rights and choices, promote self-advocacy.
• Be a custodian and mandated reporter as per policy.
