Job summary

The New York Foundling is seeking a Direct Support Professional to empower individuals with developmental disabilities. You will provide personalized support, supervision and daily living assistance to promote independence and wellbeing.

Responsibilities include implementing care plans, documenting progress, facilitating community activities, and collaborating with interdisciplinary teams to ensure comprehensive, person-centered care. A valid driver’s license and safety focus are essential.

Qualifications

• High school diploma or equivalent; direct care or human services experience preferred.
• Valid NY/NJ/CT/PA driver’s license with 6 months of experience and 6 or fewer points.
• Strong interpersonal and communication skills; patience, empathy and a genuine desire to support persons with developmental disabilities.
• CPR and First Aid certification preferred; additional training in developmental disabilities or healthcare is a plus.
• Commitment to providing compassionate care and promoting dignity and respect for individuals with developmental disabilities.
• Ability to respond effectively to changing needs and environments; flexibility and problem-solving skills.
• Maintain confidentiality, uphold ethical standards, and adhere to agency policies and procedures.
• Demonstrated strong commitment to safety.

Responsibilities

• Implement and monitor plans of care for persons supported, including assessments and behavior plans.
• Provide direct care and assistance in daily living activities: personal hygiene, meals, medication administration, transportation.
• Facilitate community outings, social events and appointments to promote inclusion and independence.
• Use person-centered approaches and positive behavior support strategies to address challenging behaviors.
• Maintain records, progress notes, and incident reports; ensure regulatory compliance.
• Collaborate with healthcare professionals, therapists, and family to develop comprehensive care plans.
• Advocate for rights, preferences, and self-advocacy; act as custodian and mandated reporter as required.
• Administer medications per schedule and monitor for adverse effects.
