Qualifications

• HS diploma or equivalent required.
• Experience with ID/DD or behavioral health preferred.
• Direct support, healthcare, human services, residential, or community-based experience preferred.

Responsibilities

• Assist individuals with activities of daily living (ADLs) and community participation.
• Support development of independence and quality of life.
• Maintain documentation per policies and regulatory requirements.
• Provide trauma-informed care and crisis response as needed.
• Assist with ISP/IHP/BSP implementation and monitoring.
