The role runs 9:00am–3:30pm, Monday through Friday, with responsibilities that include assisting with personal care, daily schedules, transportation and documentation, while promoting safety and independence. The position emphasizes person-centered support, teamwork with staff, and adherence to service plans.

Qualifications

• High school diploma or GED/HSED required.
• Good written and verbal communication skills; basic computer skills.
• Ability to exercise sound judgment in decision-making.
• Must pass health assessment and maintain per regulations.
• Clearance from background checks as required.

Responsibilities

• Provide support and training per the service plan, assisting with hygiene, grooming, escort, transitions, and mealtime needs.
• Teach self-control and self-management per departmental training.
• Complete client logs and charts by end of shift or within 24 hours with supervisor approval.
• Support individuals in daily activities and arrange transportation as needed.
• Promote safety and follow all safety procedures with team and clients.
