Job summary

Prairie Community Services in Glenwood, MN is seeking a Direct Support Provider to give daily assistance and training to individuals in our residential program. You'll help with personal cares, medication administration, meals, laundry, and daily charting.

Starting wage is $16.50-$18.50 per hour. The role is on-site with an afternoon (PM) shift and requires a valid driver's license and a compassionate, person-centered approach.

Qualifications

• Must be 17 years old or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served's program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide training and assistance with daily personal cares.
• Administer medications as directed.
• Support community activities and outings.
• Assist with money management and budgeting as applicable.
• Prepare meals and assist with nutrition monitoring.
• Help with laundry and cleaning tasks.
• Document daily progress in charts and reports.
