Job summary

Prairie Community Services is seeking a Direct Support Provider in Morris, MN to supervise, care for, and ensure the safety of individuals served. The role covers daily living activities including meals, recreation, and community orientation, with a starting wage of $11.41 per hour.

You must meet age and license requirements, pass a DHS background check, have a valid driver's license, and communicate effectively in English. CPR/First Aid might be needed and daily records must be kept accurately.

Qualifications

• Minimum of 17 years of age or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver's license.
• Must be able to read, write, communicate and understand the English language while conducting company business.
• Read, comprehend, and follow each person served's program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals being served.
• Assist with daily living activities including recreation, community orientation, meal preparation, laundry, and cleaning.
• Support medication management as needed.
• Document daily program information and progress in charts.
