Job summary

Prairie Community Services seeks a Direct Support Provider to supervise, care for, and safeguard individuals served in our Morris, MN location. You will assist with daily living tasks including meals, recreation, community orientation, and medication management.

The role is part-time and on-site, offering meaningful community-based support and teamwork. The position requires English proficiency, a valid driver’s license, and the ability to follow person-centered plans.

Qualifications

• Must be at least 17 years old or as dictated by licensure.
• Must pass DHS background clearance.
• Must have valid driver’s license.
• Must read, write, communicate and understand the English language for company business.
• Read, comprehend, and follow each person served’s program related plans and procedures.
• Deliver support to the person served on principles of person-centered services.
• Administer CPR/First Aid when needed.
• Chart daily program information.

Responsibilities

• Provide supervision, care and safety to individuals being served.
• Assist with daily living activities such as meals, laundry, and cleaning.
• Support recreation and community orientation for individuals served.
• Assist with medication management.
• Document daily program information.
